The form part of the autoresponder is the what appears on the website. On most web sites you'll see a section where they request a name and email address etc to join a newsletter or request a demo etc. That is the form that collects the information for the autoresponder.
You design the autoresponder form in accordance with the information you require. This must obviously include the name and email address and any other pertinent information you require. Once you have the email address you can send either a single email reply or sequential emails scheduled for sending at regular intervals from the first contact.
